Oh, how embarassing – all that work to file for an "amended death certificate" for his father, Jack Phillips, and he forgot to take care of the funeral record.
Inquiring minds want to know how the Funeral Director could file for the amended death certificate when the funeral record clearly shows Jack's mother's maiden name as HARRISON?
The evidence is pretty damning – the amended death certificate is either a complete forgery or someone submitted false data to the State of Oklahoma.
This also adds to the overwhelming proof that Bea Elizabeth Phillips' delayed filing death certificate is also a forgery or the result of fraudulent data submitted to the State of Oklahoma.
